An ADVANCE DIRECTIVE is a legal document that protects your health care preferences when you can’t express them. It lets you:
- Specify your preferences for health care treatment including ECT (electroconvulsive therapy) and medications.
- Designate someone to make decisions on your behalf.
- Indicate who you would like as a conservator if one is needed.

Advance Directives have many different parts. You don’t need to do them all.
Advance Directives are different from a WRAP (Wellness Recovery Action Plan) which does not provide the same legal protection. It is relatively simple to do an advance directive if you have a WRAP.
Advance Directives should be reviewed and updated annually.
